Updates from the Practice Team

Jul 22 2015

The surgery will be activating the "Shared Care Record" during August.  This new system enables health providers to safely and easily access a summary record of patient information through a secure connection via ManageMyHealth. Only authorised health professionals can access patient information.

The Shared Care Record will help health professionals involved in your care access to relevant information from your general practice health record. This will provide those involved in your care with more information, which should lead to safer and higher quality decisions than would otherwise be possible.

Additionally , in situations of civil emergencies (like earthquakes, floods or pandemics) your basic health information will also be immediately available if it is required, even if the surgery is not accessible, because your Shared Care Record is stored outside the surgery.

The information that is shared may include: prescribed regular medications , your current health problems (diagnoses and conditions), allergies, immunisations, recalls and laboratory results.

The consultation notes written by your doctor as part of any normal visit are not currently shared as part of this scheme.

You may chose to opt-out of having a Shared Care Record. If this is the case please contact the surgery and speak with reception staff who will provide you with more information.

If you have any concerns please contact the surgery.
